2021-22 Basketball Registration

Registration for the 2021-22 basketball season is open and will close Thursday, Oct. 10, 2014. Please complete the Registration Form and Consent and Liability Waiver, which can now be downloaded in one, 4-page document. 

APPLICATIONS WILL NOT BE PROCESSED WITHOUT RECEIPT OF CORRECT PAYMENT AND THE COMPLETION OF FORMS.

If you are a non-parishioner, you must also download and complete a Pastor's Appeal Form (if applicable).

Completed forms may be e-mailed to [email protected], faxed to (864) 297-5804, or delivered/mailed to the church at 2252 Woodruff Road, Simpsonville, SC 29681.

Announcement of Price Increase

Due to increases in operating costs, we must unfortunately raise our fees for the 2014-15 basketball season. However, you can lock-in last year's rate with our Early Bird Special through September 3, 2014. 

Youth Parishioner: $45 by Sept. 3, $50 by Oct. 10, $60 (late)
Men (All): $50 by Sept. 3, $55 by Oct. 10, $60 (late)
Non-parishioner: $50 by Sept. 3, $55 by Oct. 10, $60 (late)

NOTE: Please hold the $60 late registration fee until you have been notified that your child has been placed on a team.

Want to learn more? Check out this short video from last season. St. Mary Magdalene's basketball program dressed 20 teams, making it the largest Catholic basketball program in Greenville County. There were more than 200 players ranging from ages 5 to 50.

Registration Form

The registration form is to be completed per participating family. If you are an adult registering for men's basketball without any children, please list your spouse or next of kin's information under "mother."

This year we've included the Parental/Guardian Consent Form and Liability Waiver in the same download. If you are an adult registering for men's basketball, you must fill out all four forms. List your spouse or next of kin as "Parent/Guardian."

Pastor Appeals Form

If you currently attend another church but wish to play basketball for St. Mary Magdalene Catholic Church, you must have your pastor complete this form. If your church has a basketball program, you cannot participate in our league. 

 If you do not belong to a church, you must still complete this form. Please list your honest intentions on the form, such as "Just moved here looking for a church" or "Currently exploring St. Mary Magdalene."
