RegistrationThe registration form is to be completed per participating family by both new and returning players. For your convenience, you can register up to three members of your family using one form.

Consent and Liability WaiverA Parental/Guardian Consent Form and Liability Waiver must be completed for each participant. NOTE: Even though you are registering for adult softball, you must still complete these forms. Please list your spouse or next of kin as "Parent/Guardian."
